    Born c. 1816 in Massachusetts; active in Mont Vernon, New Hampshire, by 1850; in Providence, Rhode Island, 1847.

    It is not clear if Samuel O. LaForrest and Samuel Forrest are the same person or two individuals with similar names. If both names refer to the same person, LaForrest is the likely correct form. —Ed.
