Identified through online information from Jeff Scofield. -- Retained the case from E. & G. G. Hook Op. 309; partially rebuilt with tonal alterations in 1967 by Berkshire; completely rebuilt 1983-1987 by Theodore Gilbert & Associates, with further tonal alterations.
