Database Manager on February 7th, 2011:

Identified through information from Jonathan Ortloff: -- Originally installed in the Marbro Theatre, Chicago in 1927. One of only five five-manual Wurlitzers built. Installed here in basement chambers, speaking through grilles at the top into living and dining rooms. Console in living room. Relocated to Ocean State Theatre (Originally Loew's State Theatre), Providence, Rhode Island, in 1982.
